Review the results of your personal health assessment and write your realizations and plans about your health in your journal.

My personal health assessment is slightly bad because I got a low score in taking care our environment. But my plans are to improve that and practiced well. Not only the environment you should prioritized but also our physical, mental, emotional and spiritual health. All of that aspects should maintained and balance. In order for us to live healthy and feel completely inside and out.
